Application Forms
The following forms and documents must be printed, physically signed and uploaded to the student application. Click on the links below to view and download the forms.
1. Fee Acknowledgment

3. English Teacher Recommendation
This form should be signed and dated by both the student and the teacher.
4. Scan of ELTiS Test Results
Please make sure that student name, date and test score are visible when the form is uploaded.
5. Statement of Applicant Health
Please make sure that all dates are in the format mm/dd/yy for this form. It is presented to the school for proof of vaccination. Improper dates can cause confusion and delay a student's admittance.
6. Health Addendum
Please complete and upload this form for any student health issues.
7. Vaccination Agreement
The CDC's website has more information on vaccinations and the vaccination schedule.
8. Medical Emergency Treatment (HIPAA)
Please note that on this form the parent's name, date of birth and signatures should be on page 1. Page 2 is signed by a parent and the student.

13. Scan of Official School Transcript (3 most recent years)
Please make sure that this upload is clear and that the student's name, school year, class name and class final grade is clear and visible.
14. Smiling Headshot
Please make sure this is a clear photo where the student is facing the camera and you can see the student's face. We love to see happy smiling photos.

18.Scan of Passport
We use the copy of the student's passport to check student name, date of birth and place of birth for the student visa. Please make sure that a clear copy of the photo page of the student's passport is uploaded.
